WRITTEN APPLICATION
A written application should be made either by the person requiring accommodation or by the person who is responsible.

RECOMMENDATION LETTER
A recommendation letter either from the Parish Priest of the Church where the person belongs or other similar valid letter.

MEDICAL HISTORY
The latest medical profile, as well as the past history of the health issues, of the person.

FAMILY HISTORY
A brief history of the person concerned, family, children and relatives.

LIST OF ITEMS
The person must collect a list of the items to be brought at the time of admission.

STAMP PAPER RS100
The affidavit must be made on a stamp paper of Rs.100 and must be notarized by the person who is taking responsibility.

LIVING WILL
A living will of declaration to be made for medical treatment.

aPPLICATION FORM
On the day of admission, the applicant or relative must fill in the application form of the institute.

LETTER OF UNDETAKING
On the day of admission, “Letter of Undertaking” must be filled in and signed by the applicant and 2 witnesses, 1 from the applicant’s side and one from the Institute.

BIRTH PROOF
On the day of admission, the birth proof with photo identity must be produced.

PASSPORT SIZE PHOTO
On the day of admission, 4 passport size photos of the person must be submitted.

TERMS & CONDITIONS
At the time of admission, a booklet containing the “Terms & Conditions“ of the Home must be collected.
